Chronic Disease Management
Steady, compassionate support for long-term health conditions — diabetes, hypertension, thyroid disorders, and more
Living with a chronic condition doesn't mean you're on your own.
Whether you’re managing diabetes, navigating high blood pressure, balancing thyroid medication, or working on weight and metabolic health, we’re here to walk beside you every step of the way.
Chronic disease care is about more than treating numbers on a lab report. It’s about understanding how your condition affects your daily life, adjusting your treatment as your body changes, and helping you feel empowered — not overwhelmed. And with telehealth appointments available across Colorado, Illinois, and Oregon, you can access consistent, expert care without the burden of frequent office visits.
Conditions We Manage
Hypertension (High Blood Pressure)
High blood pressure often has no symptoms, but left unmanaged, it can quietly damage your heart, kidneys, and brain. The good news? With the right support and treatment, hypertension is very manageable.
Our approach includes:
- At-home blood pressure monitoring guidance (please see **** section (link to that section) for equipment recommendations and steps for accurate measurements at home)
- Medication management and adjustments
- Root cause analysis
- Lifestyle coaching for diet, exercise, and stress reduction
- Regular check-ins to track progress and prevent complications
- Screening for related conditions (cholesterol, kidney function, heart health)
You'll check your blood pressure at home before our visits, which gives us a much clearer picture of your day-to-day health than a single office reading.

Symptoms of high blood pressure: Severe headaches (often described as throbbing or, sometimes, a dull ache)
- Shortness of breath or difficulty breathing
- Vision changes
(blurry vision, double vision, or pain) - Chest pain or palpitations
- Dizziness or lightheadedness
- Nosebleeds
- Numbness or weakness
- Facial flushing or red spots in the eyes
- Confusion or mental changes
Thyroid Disorders (Hypothyroidism, Hashimoto’s)
When your thyroid isn’t working right, everything can feel off — your energy, weight, mood, and sleep. Finding the right balance with thyroid medication takes time and patience, and we’re here to help you get there.
We support you with:
- Thyroid hormone level testing (TSH, Free T3, Free T4)
- Medication adjustments and dose optimization
- Symptom management (fatigue, weight changes, hair loss, mood)
- Monitoring for thyroid nodules or changes in function
- Education on how thyroid health impacts your whole body
Thyroid care is highly individual. What works for someone else may not work for you — and that's okay. We'll keep adjusting until we find your optimal treatment.

Frequently Asked Questions
How often will I need follow-up appointments?
It depends on your condition and how stable your health is. Newly diagnosed patients or those adjusting medications may need monthly visits. Once stable, many patients check in every 3-6 months. We’ll create a schedule that works for you.
Can you manage my diabetes if I'm on insulin?
Yes. We manage both Type 1 and Type 2 diabetes, including patients on multiple daily injections or insulin pumps as long as your blood sugars are controlled on your current insulin regimen. We may recommend you see endocrine for insulin adjustments if your insulin requires significant adjustments. We’ll work closely with you to adjust doses, monitor A1c levels, and prevent complications.
What if my condition gets worse or I have a complication?
We monitor your health closely and adjust treatment as needed. If you develop complications or need specialized care, we’ll coordinate referrals to specialists while continuing to manage your ongoing primary care.
Do I need any equipment for telehealth chronic disease management?
For the best care, we recommend having a home blood pressure cuff and digital scale. If you have diabetes, a blood glucose monitor is essential. These tools help us track your progress between visits.
